If you lose your health insurance card
Health insurance card are important to also become identification. Should, in the case of lost or damaged, please take a promptly procedure below. In addition, when the lost or stolen, please report to the nearest police station.
If the old health insurance card is found, please return it to Novartis health insurance society.
- If you damage or lose your health insurance card and request to reissue of a health insurance card
- If you lose your health insurance card and cannot return it to your employer when you retire or update it
